Point-of-Sale Enrollments
 

A point-of-sale enrollment is the easiest way to use the First Script program. Simply give the injured employee a point-of-sale Prescription Authorization Form (PAF) and instruct them to give it to the pharmacist when they fill their work-related injury prescriptions.

The PAF needs to include the employee’s name, social security number, date of injury and the name of the school district where the employee works.

First Script has provided a web site for printing PAFs at www.firstscript.com/merced. You can choose to generate a form online, in which case all of the above information will be pre-printed on the form, or you can download a form in Abobe PDF format and write in the information.

After you have given the PAF form to the employee, your role in the point-of-sale process is complete.

When the employee fills their prescriptions, the pharmacy will call the toll free number printed on the PAF. This number is available 24 hours a day, 7 days a week. After receiving authorization from our Help Desk, which only takes a few moments, the pharmacy will be able to fill the prescriptions with no out-of-pocket expense to the employee. You will immediately receive an e-mail informing you that the employee has been enrolled. This is an informational e-mail only and you do not need to respond.

For ongoing management of the employee’s enrollment, including filling additional prescriptions, the enrollment is automatically transferred to the CMI adjuster responsible for your district.
